Cortisol and the Architecture of Sleep

Sleep is one of the first things people notice changing through the transition, and one of the last things a standard appointment has time to address well. This lecture connects the two systems that most often explain it: the hormonal shifts of perimenopause and the daily rhythm of cortisol that governs when you feel alert and when you wind down.

Cortisol is not the stress hormone of common shorthand. It is a timing signal. In a well-regulated rhythm it rises sharply in the first hour after waking and declines across the day to a low point at night, and that descent is part of what allows sleep to consolidate. When the curve flattens or the evening value creeps upward, the night fragments — not because you are doing anything wrong, but because the signal that should be quieting down is still speaking.

The transition affects this in two directions at once. Falling and fluctuating estradiol changes thermoregulation and sleep continuity directly, while the same fluctuations can disturb the cortisol rhythm that organizes the night. The result is a feedback that is easy to experience as a personal failing and far better understood as a measurable, treatable pattern.
